Effective July 1, 2007 our Dental Plan is with Delta Dental Insurance Company.
Everyone has a separate Dental ID card. For contact information click here .
Prior to July 1, 2007:
- If you are in Allegiance Benefit Plan Management (for your Medical plan), use the same card for Medical and Dental call 1-877-778-8600.
- If you are in New West Managed Care (for Medical plan), you need a separate Allegiance Benefit Plan Management Dental Card call 1-877-778-8600.
- If you are in Blue Cross Managed Care (for Medical plan), you need a separate Allegiance Dental card call 1-877-778-8600.
