PLEASE NOTE:

Enrollment for this Plan Year is called Annual Benefits Election as our plan now has closed enrollment until further notice.

  • No dependents (spouse/adult dependent, children) can be added to your plan unless there is a qualifying event.
  • Dropping a dependent (spouse/adult dependent, children) at Annual Benefits Election is allowed.  Once removed, dependents will not be reinstated unless there is a qualifying event.

Qualifying Event- A qualifying event  is a change in status such as marriage or divorce,  the birth or adoption of a child, or a loss/gain of eligibility for insurance coverage.
